- Supervisor holds individual Final Performance Review Session for 2008.
- PRD process workshop Tuesday Jan. 06, 2009 10am-12pm and Jan. 07, 2009 10am-12pm "Clearing the Fog".
Supervisors-
- Completes own self-evaluation.
- Determine the rating the library staff member should receive for each performance area. Write comments under each factor or area to support your assigned rating.
- Determine an overall performance rating using categories on the form.
Staff-
- Staff members complete Self-Evaluation.
Both- Supervisors and Staff
- Review the results of the last Performance Review or Midway Feedback Session conducted (action plans, training objectives, performance improvement goals, etc.)
- Review the PRD form, factors, and their definitions.
- Review the expectations set at the beginning of the Performance Review period of 2008.
- Review expectations that have been amended or revised throughout the review period.
- Gather documentation to support or define both positive and negative performance incidents. "Praise" should be documented as well as negative feedback.
- Identify areas of strength and areas in need of improvement and enhancement.
Final Performance Review format:
- Identify and explore accomplishments and concerns.
- Discuss evaluations and finalize Rating. (Note: New rating system for non-exempt employees).
- Develop or Update an Improvement or Enhancement Plan.
- Set date/time for Expectation-Setting meeting for next review
cycle.
- Implement the Improvement Plan.

- Supervisors complete merit assessments for staff and turn it in to the division's Director.
