  • Preparation for Final Review.

  • Supervisor holds individual Final Performance Review Session for 2007.
Supervisors-
  • Completes own self-evaluation.

  • Determine the rating the library staff member should receive for each performance area. Write comments under each factor or area to support your assigned rating.

  • Determine an overall performance rating using categories on the form.

Staff-
  • Staff members complete Self-Evaluation.

Both- Supervisors and Staff
  1. Review the results of the last Performance Review or Midway Feedback Session conducted (action plans, training objectives, performance improvement goals, etc.)

  2. Review the PRD form, factors, and their definitions.

  3. Review the expectations set at the beginning of the Performance Review period of 2007.

  4. Review expectations that have been amended or revised throughout the review period.

  5. Gather documentation to support or define both positive and negative performance incidents. "Praise" should be documented as well as negative feedback.

  6. Identify areas of strength and areas in need of improvement and enhancement.


Final Performance Review format:
  1. Identify and explore accomplishments and concerns.

  2. Discuss evaluations and finalize Rating.

  3. Develop or Update an Improvement or Enhancement Plan.

  4. Set date/time for Expectation-Setting meeting for next review cycle.

  5. Implement the Improvement Plan.
Deadline:
February 22, 2008.
  • Supervisors complete merit assessments for staff and turn it in to the division's Director.
Feb. 2008
Deadline: February 28, 2008.
  • All Review Sessions are to be completed and Appraisal forms turned in to Human Resources.

  • Beginning of a new Review Cycle. Identify goals and objective for current years.
 
March 2008  
Deadline:
March 21, 2008.

Divisional review and ranking must be completed.

April 2008  
April 15, 2008.

Library Executive Council (LEC) will complete its Library wide review and ranking.

July 2008
(Fiscal Year 2009)
(FY '09)
Deadline: July 1-31, 2008.

Library Staff
  • Midway Feedback and Self-Assessment.

    1. Midway Feedback Session. Identify and explore accomplishments and concerns.

    2. Develop or update an Improvement Plan.

    3. Implement the Improvement Plan.

FY09 merit awards distributed.

Dec. 2008
Deadline: December 31, 2008.
