External Loan Form (Classroom Use)

Use this form to request material to be taken out from the libraries. Items are on a first-come, first-served basis. Material is generally allowed to be taken out by Instructors for 2-3 days, and for Students on the day of their presentation only. Materials cannot be renewed online.
